The link below is the default form used to contact a requester, the only part of the link that needs to be correct is the requesters ID.
    Code:
    https://www.mturk.com/mturk/contact?subject=Regarding+Amazon+Mechanical+Turk+HIT&requesterId=REPLACEWITHREQUESTERID
If you wish to shorten the link down this will also work and should automatically add the subject section.
    Code:
    https://www.mturk.com/mturk/contact?requesterId=REPLACEWITHREQUESTERID
There are many resources available to find a requester ID if you so need it. TurkOpticon (TO) is a great source to search using HIT names, requester names, HIT ID, etc. If you have submitted a HIT previously for that requester you can do your own search using HIT Databases functionality or even find the requester under the "Your HIT status" section of your worker dashboard.
